Enrolling your child in Orange County Public Schools is now fully online.

Prior to beginning the Student Enrollment Application, please review the School Enrollment Information Checklist below to ensure you have ALL required documents ready to upload on your application.

SCHOOL ENROLLMENT INFORMATION CHECKLIST

• Verification of Legal Name
  • Birth Certificate
     
• Verification of Age* (with one of the following):
  • Birth Certificate or Passport
     
• Verification of Immunization and Physical Exam
  • Proof of immunizations on a Form 680 or 681
  • Proof of physical examination by a U.S. doctor within a year of enrollment (first day of entry at school).
     
• Verification of Academic History
  • Transcript and/or Last Report Card from previous grade (and current transcript if joining the school mid-year)
    - Child is entering 6^th grade? We need 5^th grade final report card or transcript and current 6^th grade transcript 
    - Child is entering 7^th grade? We need 6^th grade final report card or transcript and current 7^th grade transcript
    - Child is entering 8^th grade? We need 7^th grade final report card or transcript and current 8^th grade transcript
  • Authorization for Release of Information Form
  • Horizon West Middle School Course Request Form for the applicable grade
    - 6th grade HWMS Course Request Form
    - 7th grade HWMS Course Request Form
    - 8th grade HWMS Course Request Form
     
• Verification of Special education information (if applicable)
  • Current IEP or Current 504 plan
     

Please pay special attention to the requirements for Verification of Residence and Verification of Guardianship. For some families, it may be necessary to visit the Office of Student Enrollment to receive the proper documentation BEFORE completing the Student Enrollment Application.

• Verification of your residence in Orange County:
  • Home ownership: Current Homestead Exemption Card, current property tax statement, closing contract, or warranty deed along with current utility bill, ID with current address, and current mail.
  • Renting/leasing: Current signed lease, ID with current address, current utility bill, rent receipts, current mail, and renters’ insurance.
  • Living with someone who owns or rents the home: Verification of Residence from Student Enrollment (make an appointment: Appointments Link)
    • Required Documents for a verification of residence at: Verification of Residence Link

    • More Documentation may be required depending on circumstances in a case-by-case situation.
       
• Verification of Guardianship
  • Birth Certificate
  • Parent/Guardian State of Government Issued Photo ID
  • If applicable, you must provide one of the following:
    • Court Documentation (such as divorce decrees w/parenting plan or the placement of children though court)
    • OCPS Educational Guardianship / Parent Affidavit
